What an EOB is, and what it is not
After a provider submits a claim, your insurer processes it and sends you an Explanation of Benefits. It comes from your insurance company, not from the doctor or hospital, and it explains how the claim was handled.
The EOB is not a bill. Your provider sends the bill separately. CMS also notes that the EOB shows what you owe, not whether you have already paid, so keep your receipts.
